I accept ownership of the above mentioned animal, in return for which I agree to the following terms of adoption.

To house the animal in my home as a companion and not to sell, abandon, or give away.

To abide by the laws related to animal ownership in my area.

To provide the animal proper and sufficient veterinary care, housing, food, water, exercise, training, and kind treatment.

To keep identification tags on dogs at all times

Will not de-claw cat, will not crop ears or dock tail of dog

Not to permit the animal to run at large or become a public nuisance.

To notify MCP Inc if the animal is lost or stolen.

To return the animalto MCP Inc iffor any reason I cannot or will not keep it.

I accept this animal as is and understand that the information provided to MCP Inc about the animal may have been provided by third parties and MCP Inc does not guarantee the accuracy of such information. Failure to comply with any of the contract provisions entitles MCP Inc to terminate this contract. MCP Inc may then repossess the animal. The adopter will pay any and all court costs if any enforcement of this contract requires such action

I ASSUME ALL RISKS OF OWNERSHIP OF THE ANIMAL, INCLUDING ANY AND ALL LIABILITY FOR DAMAGE OR INJURY CAUSED BY THE ANIMAL.
